Patients taking part in this Home Blood Pressure Monitoring scheme should have been provided with an above-the-elbow blood pressure monitoring device by their GP practice. If any patients would prefer to purchase their own machine, The British Hypertension Society has information about validated blood pressure monitors available to buy.
If you are able to monitor your blood pressure at home, we ask that you do so:
- Twice each morning (five minutes apart)
- Twice each evening (five minutes apart)

Once you have taken your readings, please download and complete this Blood Pressure monitoring form.
You can complete this form online and return by email to your practice, or print it off and take a picture that you can return by text or email. You can also drop a paper copy off at your GP surgery.
